Transcript Requests

Current students or former students may order copies of their academic coursework at MUW by sending a written, signed and dated request to the Office of the Registrar. A charge of $5.00, payable in advance, will be made for each copy. Transcripts are not released for students that have not settled all University accounts. Your written request should include the following information:

Full name

Date of birth

Social security number

Approximate date of last attendance or date of degree

Current address and phone number

Address(es) where your transcript should be mailed

Your signature (required by the Family Educational Rights and Privacy Act of 1974)

Payment by check -- $5.00 per copy made payable to Mississippi University for Women (Note: Do not send cash.)

Payment by credit/debit card (Visa, Master Card or Discover only) $5.00 The following information must be provided with the request:
                   Name on Card
                   Credit Card Number
                   Expiration Date
                   V code -- (three digit number on back of card)

NOTE: If you need your transcript in a sealed envelope, you must specify this in your request.
