TITLE: Identification Cards

PURPOSE:  To establish university policy regarding faculty, staff, and student identification cards.

OPERATING DETAILS: 

Faculty/Staff

1. The Department of Human Resources will issue a photograph identification card to “benefits-eligible” faculty and staff during the in-processing proceedings.  Temporary employees are not issued an identification card.  The identification card serves to identify employees of the University for check cashing purposes, admission to or the purchase of tickets to certain University functions, and for other purposes.

2. Lost faculty/staff identification cards should be reported to the Department of Human Resources, and arrangements will be made for a replacement card. Identification cards remain the property of the University.  Upon termination, identification cards must be surrendered to the Department of Human Resources.

Students

1. All students enrolled will be issued a photograph identification card.   Student identification cards must be validated annually in the designated office and/or during registration.

2. Anyone student who loses an identification card will report this loss to the Department of Public Safety MUW Police Department. There is a replacement fee assessed for lost or stolen cards.

3. Students must possess, at all times, a valid student identification card and must surrender the ID card to any university official upon proper request.

4.  Students must relinquish their card when withdrawing from the university.  Employees will relinquish identification cards when leaving the university’s employ.
 

Use of another person’s I.D. constitutes fraud and may result in disciplinary action.
