Verification Process & Helpful Hints
Students Selected for Verification:
The federal
government and Mississippi University for
Women annually select a percentage of
financial aid applications for verification.
Through this process, applicants for
financial aid are asked to verify the
information contained in their financial aid
application(s). This process is determined
by random selection or by the use of certain
government-determined edits. The federal
government or Mississippi University for
Women has selected your application for
verification.
So that we may complete the
verification process, please provide the
information indicated below and return the
requested information with this form to our
office immediately. We will not be able to
proceed with your financial aid eligibility
review or to disburse any federal or
institutional aid until this process is
complete. Should you withdraw from the
University before your financial aid file is
verified, corrected (if necessary),
reviewed, and before you have been awarded,
you may not be eligible for financial
assistance.
WHAT
SHOULD YOU DO?
1.
Collect your and your parents’ signed
federal tax returns.
2. Fill
in and sign the worksheet--you and your
parents or spouse.
3. Mail
the completed worksheet and the signed
copies of the tax returns to the
financial aid office immediately for
review.
Your
school must review the requested
information, under the financial aid
program rules (CFR Title 34, Part 668).
Helpful Reminders
For Students selected for Verification
1. If you or your
parents do not have a copy of the
federal tax return, you can call
1-800-829-1040 to request a Letter
1722 or RTFTP printout.
2. If you or your parents did not (and
will not) file taxes for the previous
year, then Section D of this Verification
Worksheet must be completed.
3. If you reported earnings from the
Federal Work Study program on your
taxes, list that amount in C-3 and
identify it as Federal Work Study
earnings.
4. Taxable financial aid is the amount
of scholarships and grants that you
received during the calendar year that
exceeds the cost of tuition, fees,
books, and required supplies. The amount
spent on books can be counted only if
you still have the receipts. This amount
should be reflected
on C-3.
5. In Section B, please be sure to list
all family members that will live in the
home and will receive more than fifty
percent (50%) of their support from your
parents. It is very important to fill
out this section completely and not to
leave anything blank.
6. Make sure that all of the tax forms
are signed and that the Verification
Worksheet has all of the necessary
signatures.
7. If you or your parents made payments
to a 401(k) or 403(b) plan or received a
housing or living allowance in, please
include that amount in D-2 as other
untaxed income.
8. Make sure the student’s name and
social security number are on the top of
each page of the tax returns.
9. Remember that failure to complete
the verification worksheet in its
entirety and to submit the required
documentation in a timely manner can
result in a delay or reduction of your
financial assistance.
10. Please understand that verification
can reveal other conflicting information
that the university is required to
resolve.
