In order to receive a Federal Stafford Loan our office requires you to submit the following information. If you have any questions about borrowing Federal Student Loans please contact our office and we will assist you in financing your education. Thank you and good luck.

First-time borrowers must complete loan entrance counseling. See the link at the end of the selection process.

Note: AFTER you SUBMIT the form, you will need to continue on this page to do Entrance Counseling and sign a Promissory Note.

Students who borrow under the Stafford Student Loan Program are required to complete loan counseling BEFORE receiving loan proceeds from their first loan. Students should click here to complete Entrance Counseling

Students may also need to E-sign their Master Promissory Note. Funds will not be delivered to MUW without the entrance counseling and promissory note completion by new borrowers. Go to our Lender List and click on the "Apply Now" button to complete the promissory note. *These are one-time processes and will not be required each year a subsequent loan is obtained.

Note: If you did not see your Lender in the list provided, send an email with your name, SS#, your Lender choice, and the lender's code number. We will process your request as soon as possible. Compare rates, benefits and processing timelines before selecting a lender. Lenders who are not currently on the list of lenders above are not set up for electronic processing, loan counseling and signing promissory notes and may not be able to deliver funds on a timely basis. You need to arrange to pay all MUW fees in advance or arrange for an "extended payment plan" and then be reimbursed when loan funds are received.
